The Annual Church Profile is an annual process of collecting statistical and biographical information from churches and associations.  As the Heartland Church Network seeks to better support our churches, having accurate up-to-date information is vital. The ACP information helps us effectively plan events, prepare resources, allocate ministry funds and evaluate needs.
Every year, each church is greatly encouraged to submit just 5 data points. You can submit your information directly through the following link to the SBC Workspace. Or you can simply use the button below to fill out a quick form!

Please be sure to have the following information ready to complete the survey.
  • Church Contact Information (name, mailing address, email, phone number)
  • Primary Contact Information (name, email, phone number)
  • Total Members
  • Total Baptisms
  • Average Worship Attendance
  • Average Weekly Sunday School/Life Group Attendance
  • Total Receipts​
